It follows a review of the Oxford Heart Centre at the John Radcliffe Hospital, which came after staff raised concerns over practices.
The standard of management has been in focus and there have been claims that junior doctors lacked proper supervision.
The centre is being sued by the parents of Safeena Begum. The two-year-old from Aylesbury, Bucks died at the unit last year.
A news conference was being held today following publication of the report.
