Three pregnant women were wrongly told that their babies had died after being given ultrasound scans at University Hospital, Cardiff, a public inquiry was told. One of the women, Marina Jones, 25, was twice given a faulty diagnosis - the first time in 1990 and again in October last year. The inquiry is expected to make recommendations that may lead to procedural changes throughout the NHS.
