Six Chelsea players made the team, and the Blues take on Arsenal this afternoon.

Should Jose Mourinho's side win at the Emirates, then all they require is a win against Leicester on Wednesday and they will be crowned champions.

Hazard, a frontrunner for the PFA Player of the Year award tonight, has been named in every Premier League PFA Team of the Year since he moved to England in 2012.

Kane, meanwhile, has scored 20 Premier League goals this season. The league's top scorer, Sergio Aguero, however, joins Fabregas on the absentee list.

The Manchester City striker has score 21 league goals this season.
