Eden Hazard is set to miss Chelsea's final match of the season after having emergency dental surgery to remove three wisdom teeth.

The 24-year-old has started every Premier League game for Chelsea in this campaign, missing just 41 minutes of football in total for the Blues.

The Belgian's thrilling performances and 14 league goals played a key role in Jose Mourinho's side being crowned champions and earned Hazard the PFA Players' Player of the Year award and the FWA Footballer of the Year award, which he will now be unable to collect at the ceremony tonight.

In a statement, the FWA said: "Previous winner Gianfranco Zola will pay tribute to Hazard at tonight's function and Chelsea chairman Bruce Buck will deliver a message from the player.

"The surgery means Hazard will not feature in Chelsea's final game against Sunderland."

A win for Chelsea at Stamford Bridge on Sunday will take them to 87 points, taking them one beyond Manchester City's championship-winning tally last season. The Black Cats secured their Premier League status with their goalless draw against Arsenal on Wednesday.
