A hospital has ordered an inquiry after a 14-year-old girl, Janine Connor, from Ashton-under-Lyne, Greater Manchester, died after what should have been a simple operation to pin back her ears at Withington Hospital, in Manchester.

Her lungs collapsed under a general anaesthetic and she died in the Royal Manchester Children's Hospital six days later.
