A hospital is starting disciplinary action against a consultant claiming a patient died after a series of alleged blunders. Three other patients are said to have had unnecessary treatment because of misdiagnoses by Dr Peter Needham at Glan Clwyd Hospital, near Rhyl, Clwyd.
