A FAULTY brake on a fairground ride was blamed by the Health and Safety Executive for an accident last weekend at Porthcawl, Mid Glamorgan, which injured seven children aged 14 to 16 who were thrown from the 'Top Flip' ride when it suddenly restarted as they climbed off.
