AN independent inquiry is to be launched into the death of a girl, nine, at the John Radcliffe Hospital, Oxford, last November. An inquest recorded a verdict of death by misadventure on Lexie McConnell and said side-effects of prednisolone, a steroid drug prescribed for an eye complaint, were to blame.
