Confusion and poor communication on the bridge of the Queen Elizabeth 2 contributed to the grounding of the vessel off Martha's Vineyard last summer, a federal review board said yesterday, AP reports from Washington.
The National Transportation Safety Board said the crew of the Cunard liner and a harbor pilot from Newport, Rhode Island, made course changes without realising that the new track would take the vessel over a shoal.
