Miami Heat fired their coach, Alvin Gentry, after a late-season slump cost the team an NBA play-off berth, while the Denver Nuggets' coach, Bernie Bickerstaff, was fined $10,000 (£6,200) by the NBA for abusing the referees during his team's season-ending loss on Tuesday.
