DECCA Aitkenhead is mistaken in stating "it is almost impossible to overdose on this drug [cocaine] alone" ("The thin white line reveals its dark edge", 12 November). As an anaesthetist I administer the drug for its local-anaesthetic effects and I am very aware of its potential for triggering malignant cardiac arrhythmia and cardiovascular catastrophe in overdose.
